Training for adults

1. I am aware that MMA training involves physical contact and intensive physical exertion, which may result in injuries. I participate in the training entirely at my own risk and confirm that I will not make any claims against the club, coaches, or staff regarding possible injuries or damages.
2. I confirm that my health condition allows me to participate in physical activities, and I commit to informing the coach of any potential health risks or limitations prior to training.
3. I am aware and agree that during training I am obliged to follow the club's internal rules, the coaches' instructions, and behave in a way that does not endanger myself or other participants.

Training for children

Confirmed by parent

1. As a parent or guardian, I confirm that my child participates in MMA training with my consent and knowledge. I understand that due to the specific nature of contact sports, injuries may occur during training. I am aware of the risks and confirm that my child participates in the training at my own risk. The club organizing the training, coaches, and staff are not responsible for any possible injuries or other damages that may occur during or as a result of the training.
2. I confirm that my child's health condition allows them to participate in physical activities, including contact sports.
3. I understand that to ensure the safety of the training, my child and I are obliged to follow the club's internal rules and the coach's instructions.
